Health Perks benefits will now offer $100 more than in previous years, for a total of $250 in product incentives1 available to members – a $150 reward for visiting their primary care provider (PCP), $50 for account-related actions, and $50 for completing a Health Risk Assessment (HRA). Earning their $250 reward is simple, and funds are good for 365 days after the member redeems a reward. The required tasks include:

1. Visiting their PCP, for any reason

2. Registering for an online account at myuhc.com/exchange

3. Opting in for all types of text alerts

4. Signing up for all paperless communications (must opt-in to all email types)

5. Setting up Autopay to pay their monthly premium

6. Completing the Health Risk Assessment on myuhc.com/exchange
