What you need to know about SSBCI

• Help members self-indicate their qualifying chronic condition on the UnitedHealthcare mobile app or member site or by calling the Member Services number on the back of their ID card.
• When using third-party enrollment tools, such as Sunfire and Connecture, you are now required to answer the question related to chronic conditions. If the client does not have a condition, you may select “None of these conditions apply or I choose not to answer” and proceed without listing a provider and consenting to the HIPAA authorization.
